Bring with you:

1. A copy of your SR22 from your insurance company.  If you do not have a copy of your SR22 you may fax it in within 30 days.

2. The vehicle registration that the equipment is being installed into. If the vehicle is registered to any one other than the driver, a letter of authorization must be submitted by the registered owner.

3. Picture identification (passport, Colorado Identification Card, Military
Identification, Invalid Driver's Licence, School/Employment Identification Card) of the driver of the vehicle. 

4. Letter of Reinstatement from Colorado Department of Motor Vehicles.
